Our future health care system depends on both votes. The Supreme Court may impose boundaries on the exercise of government power, but voters will decide whether government takes any responsibility for the health of its people.
Thus, the ACA has been a qualified success. It did not restructure the health care or insurance systems. Rather, it made incremental changes, enabling new private insurance options with federal subsidies and expanding Medicaid with 90 percent (initially 100 percent) federal financing. Public health improved by several measures. To some, the ACA represents a progressive step toward social insurance and social solidarity. To others, it symbolizes federal overreach and promises higher taxes. Both groups are counting on either the Supreme Court or the next election to support their position.
It cannot be stated strongly enough that your vote both directly and indirectly influences healthcare. You can probably see evidence of this in your day-to-day life as well as in the broader healthcare systems in the US. Just remember, your vote:

Compared to the likelihood of a vote changing the outcome of an election, how much it matters who wins i) is harder to quantify, ii) depends more on your values, and iii) varies widely depending on the candidates running for office. But a quick scan of the numbers and issues at stake suggests that the impact will often be substantial.
If, having compared the candidates and the closeness of the election, you think a vote for the right person is in some sense worth thousands of dollars, that sounds pretty good. However, it has to be compared to the best alternative ways to use your money to improve the world, which may also offer a huge return on investment.
In a situation like that, the hour you spend voting is likely to be the most impactful one in your entire year, and could on average get you some influence over how hundreds of thousands or millions of dollars are spent. For this reason I vote whenever I get the chance.

The Paul Wellstone and Pete Domenici Mental Health Parity and Addiction Equity Act of 2008 (MHPAEA) is a federal law that generally prevents group health plans and health insurance issuers that provide mental health or substance use disorder (MH/SUD) benefits from imposing less favorable benefit limitations on those benefits than on medical/surgical benefits.
The Mental Health Parity Act of 1996 (MHPA) provided that large group health plans cannot impose annual or lifetime dollar limits on mental health benefits that are less favorable than any such limits imposed on medical/surgical benefits.
MHPAEA preserves the MHPA protections and adds significant new protections, such as extending the parity requirements to substance use disorders. Although the law requires a general equivalence in the way MH/SUD and medical/surgical benefits are treated with respect to annual and lifetime dollar limits, financial requirements and treatment limitations, MHPAEA does NOT require large group health plans or health insurance issuers to cover MH/SUD benefits. The law's requirements apply only to large group health plans and health insurance issuers that choose to include MH/SUD benefits in their benefit packages. However, the Affordable Care Act builds on MHPAEA and requires coverage of mental health and substance use disorder services as one of ten EHB categories in non-grandfathered individual and small group plans.
The final regulation applies to non-Federal governmental plans with more than 50 employees, and to group health plans of private employers with more than 50 employees. It also applies to health insurance coverage in the individual health insurance market. It does not apply to group health plans of small employers (except as noted above in connection with the EHB requirements). Like the statute, it does not require group health plans to provide MH/SUD benefits. If they do, however, the financial requirements and treatment limitations that apply to MH/SUD benefits cannot be more restrictive than the predominant requirements and limitations that apply to substantially all of the medical/surgical benefits.
Medicare, Medicaid, and the Children's Health Insurance Program (CHIP) are not group health plans or issuers of health insurance. They are public health plans through which individuals obtain health coverage. However, provisions of the Social Security Act that govern CHIP plans, Medicaid benchmark benefit plans, and managed care plans that contract with State Medicaid programs to provide services require compliance with certain requirements of MHPAEA. To protect water quality for future generations, Cape Codders face a $4 billion price tag to clean up nitrogen pollution in our embankments and estuaries. I drafted and passed legislation to establish the Cape Cod & Islands Water Protection Fund to provide tens of millions in state relief for wastewater projects across the region. In its first two years, the fund has already realized millions of dollars in property tax relief. Environmentalists and business leaders have hailed it as the most significant legislation for the region in a generation. Forever chemicals are polluting our drinking water and pose a threat to our health. I led an interagency task force, put forward a roadmap to phase out PFAS and ban its sale in Massachusetts, and secured $250,000 to fund a firefighting foam take-back program to protect the health of our firefighters. I am also working to keep Holtec from dumping radioactive water in Cape Cod Bay.
The future of work is evolving every day. Where businesses go next depends on the choices made now. Approaches that have worked in the past are based on outdated concepts about people and business. Leaders must reimagine a new model that centers on people and how, where and why they work. Business leaders can begin to change the current trajectory and shape a future of work that benefits both workers and the business. These questions are a good place to start:
No one could have predicted or altered the course of events that have changed work as we know it. We can, however, decide how to respond and improve work for the future. Our Future of Work 2021 and 2022 research show that productivity and well-being are not contradictory, but complementary. People need access to resources that support their fundamental human needs, and they should be able to work in a supportive environment, regardless of their physical location. Those who will lead in the future of work will be the ones who are creating a foundation for healthy, effective workers and organizations that not only survive, but thrive in times of change.
On March 15, 2021, almost exactly a year after ordering schools to close, Gov. Jay Inslee signed an emergency proclamation citing a youth mental health crisis and requiring all public schools to at least partially reopen. That same week, Centralia middle and high schools finally welcomed students back to campus.
So, my fellow Americans, whatever you may believe, whether you prefer one party or no party, whether you supported my agenda or fought as hard as you could against it -- our collective futures depends on your willingness to uphold your duties as a citizen.
